Assessment of the functional state of the myocardium in patients with essential hypertension (hypertensive disease) and hyperaldosteronism

Echocardiography with calculation of parameters of diastolic function and determination of aldosterone level in plasma were used in evaluation of 90 patients with essential hypertension and hyperaldosteronism. Criteria for inclusion in the study were diastolic blood pressure 100 mm Hg or higher and left ventricular myocardial mass (Teicholz) exceeding 200 g. There were differences between patients with essential hypertension and with hyperaldosteronism in parameters of cardiac diastolic function (phase of isovolumic relaxation 0,126±0,005 sec and 0,093±0,008 sec, left ventricular rapid filling volume fraction 26,5±3,17 and 43,6±3,42 ml in patients with hyperaldosteronism and essential hypertension, respectively). These data present indirect evidence of the effect of elevated plasma aldosterone on myocardial structure and function.
