The pre and post chemotherapy pathologic spectrum of germ cell tumors

The majority of germ cell tumors (GCT) arise within the gonads of males and females with the remainder originating along midline structures including the retroperitoneum, pineal gland and mediastinum. Mediastinal primaries account for 1% to 5% of all GCT, this being the second most common primary site. They account for 10% to 20% of all anterior mediastinal masses and arise within the thymus. One must keep in mind that mediastinal lymph nodes are a common metastatic site for advanced stage gonadal GCT, but these will usually have metastatic disease at other sites as well, particularly the retroperitoneum and lungs.
