Leukocytes are a component of the blood pool, and as such, these cells may leave the intravascular compartment during active bleeding, even in the absence of infection. Active bleeding therefore can cause a false-positive labeled leukocyte study. This pitfall was encountered recently in a 57-year-old woman who underwent a traumatic lumbar puncture 3 hours after injection of labeled white cells.
